Home Safety & Modification Evaluations

Set up a time for us to come to your home.
Contact your primary care physician to request an Occupational Therapy order. The order should read "OT to evaluate for home evaluation".
Collaborate with you to determine your current struggles, areas of concern, and goals for the future.
Evaluate the house (ie. measure doorways, look at bathroom accessibility, etc.).
Collaborate to determine a plan to address areas of concerns.

Help you identify the type of contractor, if any, to address your needs.
